*Remember These Special Dates*

Several of life’s important occasions require an update of your records at Anthem Blue Cross and Blue Shield and HealthNet.  Please notify the City of Hartford’s Benefit office about:

1. Your wedding day.  Notify the Benefits Office within 31 days of marriage.  A copy of your marriage certificate must be provided at time of enrollment of your spouse.  If your spouse is not added within the 31-day window, you will have to wait until the next open enrollment period.

 2. A change of name.  If you are changing your name, notify us as soon as possible. A
 copy of the legal document changing your name must be provided.

 3. Newborn Children.  Your Benefits office should be notified of new arrivals within 31 days of birth.  Your newborn’s birth certificate and Social Security No is required.  However, we encourage you to contact Benefits office in the interim.  If your newborn is not added within the 31-day window, you will have to wait until the next open enrollment period.

 4. Adopted Children.  Notify your Benefits office within 31 days following the child’s date of placement with you or a child for whom the court has appointed you legal guardian and who lives with you.  A copy of the legal documentation of adoption must be submitted.  If your adopted child is not added within the 31-day window, you will have to wait until the next open enrollment period.

 5. A dependent’s birthday.  When your child reaches the maximum age limit on your contract, Anthem Blue Cross and Blue Shield and HealthNet automatically will notify  you.  Check to be sure they have your correct address.  Don’t forget to apply for an extension of family coverage for a dependent child who is disabled.

 6. Your Child’s wedding.  Dependents who marry must be removed from your contract promptly.

 7. Death of a subscriber.  Surviving dependents of an Anthem Blue Cross and Blue Shield or HealthNet member may continue coverage themselves.  Contact the Benefits Office within 30 days after the member’s death.

 8. The day of divorce.  Contact the Benefits office, since your former spouse must be removed the first of the month following your divorce.

 9. The day you move.  Notify Blue Cross and/or HealthNet of your change in address by telephone or obtain from the Benefits Office Blue Cross’s or HealthNet’s Membership Change Form(s).

 10. Your Child’s Military Duty.  Dependents who are called to active duty or enlist in the military service must be removed from your coverage the first of the month following activation or enlistment.

 11. Change of Address.  Contact the Benefits office or Anthem Blue Cross and Blue Shield or HealthNet to update your address.  You are also responsible for the internal notification through Human Resources Department.

 12. Civil Union.  Notify the Benefits Office within 31 days of civil union.  A copy of your civil union certificate must be provided at time of enrollment of your partner.  If your partner is not added within the 31-day window, you will have to wait until the next open enrollment period.
