Disease Prevention and Health Promotion Division

The Disease Prevention and Health Promotion Division at the Hartford Health and Human Services Department is divided into three separate programs:

1) Communicable Disease

a)      Sexually Transmitted Disease (STD) Clinic

b)      Tuberculosis (TB) Clinic

c)      Public Health Nursing  

2) HIV

3) Metro Prevention Coalition

 

 

Sexually Transmitted Disease (STD) Clinic 

The STD Clinic offers free and confidential testing, treatment, and counseling for Gonorrhea, Chlamydia, Syphilis, Herpes, and other STDs. HIV testing and Hepatitis C screening is available. The clinic also offers Hepatitis A, B and Gardasil HPV vaccines to those who are eligible. In addition, education is provided to individuals and groups. Our services are available to persons 13 years of age or older.  All information is kept confidential to protect the privacy of the person.  

The STD clinic is conveniently located at 131 Coventry Street.  Our office is open Monday through Friday, 8:00am – 5:00pm.  No appointment is necessary; this is a free walk-in clinic. Just bring a photo ID.  Free condoms are available.

Tuberculosis (TB) Clinic

The TB clinic offers testing and follow-up health care for tuberculosis.   These services are for those persons with active TB disease, those with positive skin tests (TB infection), and their contacts.  Education regarding TB is available for individuals and groups.

The TB clinic is conveniently located at 131 Coventry Street.  The physician is available the 2nd and 4th Wednesdays of the month, 1:00pm - 3:00pm.  Follow-up with the Nurse Practitioner is every Wednesday.  Appointment is required.

Public Health Nursing

The Public Health Nurses (PHNs) work in the community to prevent the spread of contagious diseases such as tuberculosis, salmonella, shigella, and hepatitis. The PHNs make home visits to identify and to educate infected persons and their contacts. 

The clinic is conveniently located at 131 Coventry Street.  Our office is open Monday through Friday, 8:00am -5:00pm.

HIV 

This site contains HIV prevention messages that may not be appropriate for all audiences. Since HIV infection is spread primarily though sexual practices or by sharing needles, prevention messages and programs may address these topics. If you are not seeking such information or materials, please exit this Web page.  

The HIV Program offers free confidential or anonymous HIV counseling and testing using the OraQuick Rapid HIV-1/2 Antibody or Ora-Sure HIV Antibody testing.  HIV results are available in 20 minutes.   In addition the Program provides a hotline telephone service.  These services are also offered to community organizations, halfway houses, homeless shelters, soup kitchens, alternative incarnation sites, transitional housing programs, youth groups, school based clinics, drug rehabilitation programs, hearing impaired communities, and at community health fairs.

Staffs are multicultural and bilingual in English and Spanish.  Interpreters can be arranged for hearing impaired community members seeking the Program's services.   Walk-ins are accepted, but appointments are strongly recommended.

The HIV program will provide the following services to you:

Counseling and Testing  

  • Provide individual risk assessment by a certified HIV counselor  

  • Provide HIV antibody testing, results are always given in person 

  • On-site finger-stick testing in community settings

  • Presentations are available on HIV antibody testing

  • Follow-up and medical referrals  

  • Condoms and lubrication are available at all sites and FREE of charge

  • TB referrals

  • Anonymous and confidential testing

  • Referrals to other services

  • Referrals to counseling and testing services

  • Referrals to Ryan White services (case management)

  • Attend health fairs and conferences  

Hotline Education and Appointments

  • May call our offices and ask questions with or without using your name

  • Information and referrals to other AIDS/HIV services

  • Appointments made without the use of your name

  • Walk-in services are accepted, however it is best to call and ask about waiting time

  • Information can be mailed to your home

  • Risk assessment is available by telephone, hotline director will make appointment if needed

METRO PREVENTION COALITION

Project Purpose: With the funding of a SAMHSA grant the Metro Hartford Prevention Coalition (MPC) was convened with the purpose of joining two health departments and one health district representing four contiguous municipalities (Hartford, East Hartford, West Hartford and Bloomfield), in a collaborative prevention effort aimed at reducing the incidence and prevalence of substance abuse, HIV and hepatitis among people of color and the minority reentry population. In addition to the four municipalities, three state agencies (Correction, Mental Health and Addiction Services, and Public Health) and fifteen service providers have joined together to form the Coalition. 

Faced with multiple public health issues complicated by population mobility between towns, the MPC is conceived as an important strategic alliance for purposes of improved data collection and epidemiological services, joint program development using evidenced-based interventions, collective staff development opportunities, and improved effectiveness and efficiency of programs and services provided to residents of each municipality through both municipal and community-based services. The MPC will enable development, implementation and strengthening of a regional infrastructure to promote enhanced health literacy about HIV/AIDS, SA/Mental health, and Hepatitis, as well as early intervention and prevention in the target populations. Using a regional public health systems approach, this strategic collaboration will emphasize prevention and risk reduction.  

The Metro Hartford Prevention Coalition (MPC) has three major components:

  1. The community assessment component of the proposed project will enable the partnering municipalities to gain a clearer understanding of the incidence and prevalence of HIV, AIDS, and hepatitis in the communities.
  2. The community mobilization and capacity building component will allow the partnering agencies to develop a cadre of trained indigenous community health workers to conduct effective outreach and prevention for these diseases. It will also help build strong collaboration among the health districts, state agencies and community-based service providers, most of which are located in Hartford and serve a regional population.
  3. The strategic plan component will allow these communities to build upon the knowledge gained through the statewide HIV Prevention Plan, and to ensure that the evidence-based strategies that are selected and implemented are genuinely culturally appropriate.

 The foundation provided by these components will provide the partnering communities their greatest opportunities for success in reducing HIV among people of color as the interventions are implemented. In addition, the monitoring, data collection, and evaluation to be provided under the proposed funding will allow the project partners to learn valuable lessons for refining and improving their efforts. The requested funding will also be used to increase the target area’s capabilities in regard to data collection and monitoring by increasing epidemiological services for timely reporting on a town-by-town basis for Hartford, East Hartford, Bloomfield and West Hartford.
