THE FILE OF LIFE PROGRAM

 

Welcome to the “File of Life Program”

 

 

The “File of Life” consists of a red, plastic pocket that has a magnetic back that is kept on

 

 the outside of your refrigerator. Inside the pocket is a blank card that you fill out with

 

your medical information that will assist First Responders in obtaining your medical

 

history. Here are some additional tips to assist you in filling out your card:

 

 

  1. Use PENCIL when filling out the “File of Life” card, this will make it easier to update the card when future changes become necessary.

 

  1. Please make sure that you print clearly and spell all your information correctly. If you need more space to write, staple an additional piece of paper to your card.

 

  1. List your primary care Physician.

 

  1. Keep all medical information up to date.

 

  1. When completed, place the “File of Life” on the outside of your refrigerator.
